    In consideration for being permitted to participate in the BHI5 Program 2017, I hereby agree to and represent the following:
    基于被接受加入 BHI5 2017年项目，我在此声明知晓并同意以下几点：
    1. Payment of the fee and completion of required forms are evidence of my commitment to participate in the above-indicated program. I understand that withdrawal from the program for any reason will result in forfeiture of the $300 USD fee.
    1，费用的支付和相关表格的提交是阐明我参与上述项目之目的的证明。我知晓并同意在项目开始前，以任何原因退出，将导致300美元订金的无法退回。

    2. I will accept unconditionally the authority of the designated representative of BHI5 and/or partners designee throughout participation in the program.
    2，在项目期间，我无条件接受由 BHI5 指派的代表或合作者的权威。

    3. I will complete the designated period of BHI5 program (i.e., from the first formal meeting of the group to the end of the last presentation/exhibition)
    3，我会全程参与 BHI5 项目规定的活动。（例如，从开始的集体会议到最终的作品展示。）

    4. I will pay the fees of DESIGN H(ij)ACK for the entire period of the program. Financial commitments are made in advance on the assumption that participants are enrolled for the full period. It is understood and agreed that participants will provide any additional amount necessary to cover actual expenditures as well as any deficit occasioned by factors and circumstances not now known (e.g. unforeseen personal expenses, exchange rate variances).
    4，我会支付DESIGN H(ij)ACK 项目的全程费用。付费协议基于参与者会参与项目全程而设立。 BHI5 与我都同意，在项目进行中可能出现的未知因素导致的费用增加，该额外增加的费用仅以让项目正常持续运行为目的，我愿意支付因此而增加的其他费用。（例如但不限定于：无法预知的个人支出，汇率波动导致的差额。）

    5. In order to utilize fully the program and to derive the utmost benefit from the experience of Mentors, I recognize that I am required to attend all regularly scheduled meetings, trips, presentations, critics, lecture, etc...
    5，为了最好的利用项目，从导师学到最大化的知识与经验，我同意参加所有的常规会议、旅行、展示或演讲、论坛、讲座等等。

    6. I will conduct myself in such a way as to reflect credit upon bhi5 and my Country, and to undertake seriously my professional assignments in order to maintain for subsequent BHI5 participants the opportunities that my predecessors preserved for me. I understand that my behavior must be in accordance with the laws of the country in which I will reside during the BHI5 program.
    6，在项目期间，我会以自己的言行反应 BHI5 的应有荣誉，认真对待项目任务，在享有我的前任参与者留给我的遗产的同时，为我之后的参与者也留下他们应得的财富。我保证，在 BHI5 期间我的行为不会违反项目所在地国家的法律。

    7. Participants should be aware that there will not be the same type of support structure that is available in their home country. The program assumes that participants who are accepted are emotionally mature and are aware of what is considered proper behavior, and, therefore do not need specific rules for every situation but will follow their good judgment.
    ​7，我理解，参与者作为外国人来到项目所在地国，不会得到在祖国一样的社会支持。项目方默认，参与者是一个成熟的个体，知道何种行为为得体，因此不再需要时时刻刻的社会行为指导，可以按照自己的判断行事。

    8. The designated representative of BHI5 and/or partner, has the authority to terminate, at any time program participation of a participant whose continuation would be detrimental to him/herself, to others, or to the Family. The decision of termination is made at the sole discretion of the BHI5 and their decision is final. Return travel and any other expenses due to such program termination are to be paid by the participant.

    9. I authorize release of medical records held by my Health Service or any hospital, clinic, or doctor to the designated representative of BHI5 should a medical necessity arise. I, the participant, hereby authorize the designated representative of BHI5 and/or resident associate director, at her or his discretion, to notify and make full disclosure to my parent or guardian in the event of an emergency. On rare occasions, emergency requiring hospitalization and/or surgery develops. In order to prevent a dangerous delay in case of a medical emergency, the signature found below of the parent/guardian authorizes the designated representative of BHI5 to secure whatever treatment is deemed necessary for the student participant, of an anesthetic surgery.9，在出现需就医的紧急情况下，我授权向 BHI5 所派代表公开我的医疗记录和资料。我作为项目参与者，在此授权 BHI5 代表和相关驻地医生，在他们的最好判断下，在紧急情况发生时，通知我的父母或监护人并作出详细解释。在比较罕见的情况下，紧急情况可能会发展为住院或手术。为了防止情况恶化，下面的监护人亲笔签名授权 BHI5 所派代表在紧急情况下为学生参与者寻找必需的救治手段，包括麻醉手术。
    //////Housing Package during "DESIGN H(ij)ACK", I hereby agree to and represent the following: / 在 "DESIGN H(ij)ACK" 住宿问题上，我知晓并同意以下两点：
    A. I will accept the housing and living arrangements made for me by the designated representative of BHI5 and or partners of the program and to abide by the regulations governing the conduct of students in these lodgings. I may not change these lodgings during the six weeks without permission obtained in advance from BHI5 to whom full particulars are to be sent before I take any action.
    A, 我接受由 BHI5 所派代表或项目合伙人提供的住宿条件和安排，并遵守在住宿地内的行为规则。在6个星期的住宿期间，我不会在不通知 BHI5 并获得允许的情况下改变住宿安排，在我做出任何改变住宿安排的行为前，我都会向 BHI5 提供相关细节。

    B. I hereby agree, on accepting the accommodation provided by the BHI5 program, that I shall pay in full of $550 USD required throughout the period for the entire 6 weeks of the program, and that, should I vacate such accommodation before the end of that period I shall be liable for any fees or rents outstanding for the remainder of that period. I agree to pay for any damages for which I am responsible.
    B, 我在此同意，在接受 BHI5 提供的住宿的情况下，我需一次性支付550美元，该费用为6个星期的全程住宿费用。并且，如果我在项目结束前自愿放弃了该住宿条件，不会有费用退回。我同意，我将支付赔偿在住宿期间造成的损失。
    10. I agree to execute and abide by the Waiver, Release and Indemnification Agreement for participation in the above indicated program. Acknowledgement by Participant: I, ________________have read the Agreement for participation in "DESIGN H(ij)ACK" and fully understand each of the terms contained therein. I execute this agreement voluntarily and with full knowledge of its significance.
    ​10，我同意执行和遵守为上述项目参与人所撰写的《协议》。参与者声明：我，__________ 仔细阅读了《"DESIGN H(ij)ACK"参与人协议》，完全理解其内容。我自愿执行该协议，并知晓其所蕴含的意义。
Final Payment 全部学费支付:
​If you have already paid the enrollment fee please deduct $300 USD from the cost of the package you choose.
